Go to Dashboard > Products > Product Settings and find the Addon Product Display Settings section.
Choose whichever style best suits how many add-ons you offer - boxed generally looks better with a handful of visual options, while list suits a longer, simpler set of choices.
Make sure Display Addons is also switched on in the same section, otherwise add-ons will not appear on the product page regardless of the display type chosen.
